Start of Care Documentation, Cut in Half
At SDQ Healthcare Services in Mission, Texas, Lime cut Start of Care documentation time in half. Charts reach QA sooner, and medications go straight into Axxess.
Start of Care documentation time
On every Start of Care, by the estimate of Erica Villalobos, Director of Nursing
“It really truly did cut the time down in half with the documentation. That’s a huge win for us.”
Quotes are from Erica Villalobos, Director of Nursing, and Sam Rodriguez, Director of Quality, at SDQ Healthcare Services, in a conversation with Lime at the end of SDQ’s pilot.
Half the time on the heaviest visit
The Start of Care is the most documentation-heavy visit in home health. Erica ran many of SDQ’s pilot visits herself so she could be the point of contact for her team. Her verdict: Lime took about half the documentation time off every Start of Care.
Faster to QA
Clinicians stopped holding charts while they caught up on documentation, so finished work reached SDQ’s quality team earlier than it used to.
“It got to QA much faster than usual, because we weren’t delaying our charting time.”
A summary built for the reviewer
The feature Erica named first was the OASIS summary at the end of each chart, which pulls together what a reviewer has to verify before anything else.
“I liked the OASIS summary at the end, how it puts together all the important parts: the homebound status, the skilled need, the assessment that occurred, and the major findings.”
Sam, who reviews those charts, agreed:
“It’s pretty good. It’s detailed. I like it.”
Medications into Axxess, with gaps flagged
When a spoken medication was missing a detail Axxess needs, Lime flagged it before anything was sent. Erica filled in the gap, and the full list landed correctly in the medication profile.
“It flagged me, I was able to go in and add the right information, and then it uploaded everything.”
From pilot to rollout
After the pilot, SDQ moved forward with Lime. Erica is extending it to skilled nursing visits by her LVNs and to her physical therapy team.
“My PT staff, they were interested. They’re excited.”

At Purpose Driven Home Health, Lime saves about an hour on every Start of Care. It is easy enough that clinicians forget it is running, and its in-visit flags point out what could be missed before they leave the home.
