40 Minutes Back on Every Big Visit

The big notes were the ones costing him evenings

Short follow-up visits were never the problem. Admissions and residential care visits were. They carry the most documentation, they take the longest to assemble, and they are the visits most likely to follow a clinician home.

Those are the visits Zach put Lime against first.

“Lime has tremendously streamlined my entire approach to some of the bigger notes in home health — admission, residential care, that sort of thing. I’d say on average it’s saving me about 40 minutes per visit, which is huge.”

Thirty minutes of prep, removed

Most of the time Zach got back did not come out of the visit itself. It came out of the half hour before it. Pulling the chart together, reviewing history, and staging the assessment used to happen on his own time, ahead of every documentation-heavy visit.

Across his documentation overall, not only admissions, he puts the reduction at 40 to 50%.

“I’d say 40 to 50% — because for me, a lot of it’s prep work. I no longer have to spend 30 minutes before a visit getting my stuff together. Now I just have to worry about my time in the home, and a little bit on the back end.”

What remains is the visit and a short review afterward. The work between those two points moved into the software.

The notes got more accurate, not only faster

Speed is the first thing clinicians notice. Completeness is the one Zach kept coming back to. A visit produces more detail than anyone reliably recalls an hour later, and the pieces most often lost are the ones a reviewer or a payer will look for.

“I’m also more accurate with my documentation. The software has allowed me to catch things I may have forgotten in the visit and create a more accurate note — which goes back to providing better care.”

A more complete note is worth more than a faster one. It holds up under QA review, it supports the codes attached to it, and it gives the next clinician an accurate picture of the patient.

The visits nobody wanted became worth taking

This is the change Zach described last, and it is the one with the most operational weight behind it.

In most agencies, the heaviest documentation visits are the hardest to staff. Clinicians avoid them because the paperwork attached to an admission outweighs the visit itself. Schedulers feel it as pushback, and admissions sit waiting for someone willing to take them.

When the documentation cost drops, the incentive flips.

“I now look forward to these visits that have more documentation. It’s not as much of a burden anymore — there’s incentive now behind taking those visits because it doesn’t take as much time.”
