How to Read This Market in 2026

The ambient documentation market has stratified into four tiers. Enterprise health-system platforms (Abridge, Ambience, Microsoft Dragon Copilot) compete on deep Epic integration and system-wide contracts. Mid-market and specialty tools (Suki, DeepScribe, Nabla) differentiate on workflow fit and specialty accuracy. SMB tools (Freed) win on price and speed-to-value for individual clinicians. And setting-specific platforms build for documentation the generalists don't handle. In post-acute care, this is Lime Health AI, which drafts complete OASIS-E2 and HOPE assessments with ICD-10 coding rather than narrative encounter notes.

A fifth force emerged over the past year: EMR vendors embedding their own scribes. WellSky shipped WellSky Scribe (ambient transcription) in late 2025 and Homecare Homebase launched Curate: Scribe in March 2026. These are drafting tools inside a single EMR, a meaningful upgrade over templates. WellSky says its Scribe now covers all major home health visit types, including OASIS visits, and in August 2026 it folded Scribe into its SkySense AI suite at no additional cost for WellSky Home Health EHR clients. Coding review and QA remain separate jobs in both EMRs.

What Changed Since August 2026

• Abridge won federal scale and added coding review. On September 22, 2026 Abridge announced selection for the VA's ambient AI enterprise contract, a multiple-award vehicle with a $775.72 million ceiling over five years, and said it already serves VA clinicians at more than 75 medical centers. On September 14 it launched pre-bill review, which checks coded diagnoses and DRGs against the documentation for CDI and coding teams.
• Ambience changed how it charges. In August 2026 Ambience introduced performance-based contracts that put its fees at risk against each health system's outcome targets, and cited Cleveland Clinic onboarding 4,000 clinicians in four months.
• Funding kept flowing to self-serve and European scribes. Heidi Health raised $340M at a $900M valuation and Tandem Health raised a $100M Series B, both in September 2026.
• EHR vendors widened their built-in agents. Oracle Health added coding, dictation, and chart review to its Clinical AI Agent (August 19) and released it for U.S. inpatient nurses (September 14). Commure gained EU and UK clearance for its ambient product.
• Home health EMR scribes got cheaper. WellSky now includes WellSky Scribe at no additional cost for WellSky Home Health EHR clients and reports that consistent users see 8.6 more patients per clinician per month with 22% less after-hours documentation.
• Entry pricing fell. Freed's published plans now start at $39 per month, and Upheal's AI Notes plan is $39 per month with unlimited sessions.

Almost every company on this list prices per provider per month, which suits salaried physicians seeing full schedules. The reported 2026 range runs from ~$99 (Freed) to $500+ (enterprise platforms), before implementation fees. The main exception is per-chart volume pricing (Lime), where cost tracks documentation volume rather than headcount, a better fit for agencies with part-time and per-visit field staff. The Long Tail: More Ambient Documentation Vendors

Beyond the market leaders above, a broad second tier of ambient documentation vendors serves specific EHRs, specialties, and regions. Descriptions reflect public positioning as of mid-2026.

• Tandem Health: Nordic ambient scribe serving European health systems. Raised a $100M Series B led by EQT's Scaleup Europe Fund in September 2026 and reports 10,000 care organizations across 14 European markets.

None of these vendors complete structured post-acute assessments. For OASIS-E2, HOPE, and PDGM-linked coding inside HCHB, WellSky, Axxess, or MatrixCare, the purpose-built option remains OASIS Scribe.

Names That Show Up in This Search but Are Not Ambient Scribes

AI answers about ambient documentation sometimes surface healthcare software from neighboring categories. Three that come up often: Climedo is a European electronic data capture (EDC and eCOA) platform for clinical trials and public health reporting, not a scribe. Averbis is a clinical NLP company that structures text in existing documents, which sits downstream of documentation capture. Recovery Cat is a digital therapy companion for psychiatric care, a patient-facing tool rather than a documentation product. If a vendor list includes these names alongside scribes, the list was assembled by category keyword rather than by what the products do.
